At Foundation Chiropractic, we operate as a private-pay practice rather than participating in insurance networks. This allows your care recommendations to be based on your needs, not insurance company limitations.
For patients who have out-of-network benefits, we provide a detailed superbill after each visit. A superbill contains the diagnosis and procedure codes many insurance carriers require for reimbursement. Depending on your plan, you may receive partial reimbursements directly from your insurance company.
Most adult visits are approximately $95, while pediatric visits are approximately $85. If you'd like personalized pricing before starting care, we offer complimentary consultations so you can speak with a doctor and receive clear answers about costs and recommendations.

For your first appointment, please bring a valid photo ID and any relevant health information you would like the doctor to review.
If you plan to seek reimbursement through your insurance company, it can be helpful to bring your insurance member ID card and information regarding your out-of-network benefits.
After your visit, our team can provide a superbill containing the diagnosis and procedure codes commonly required for reimbursement requests. We make sure the documentation is accurate and complete so you have everything needed for submission.

Do you treat auto accident injuries?
Yes. We regularly work with patients injured in motor vehicle accidents. In many Florida cases, Personal Injury Protection (PIP) benefits can help cover treatment costs. To preserve eligibility for PIP benefits, care must generally begin within 14 days of the accident.
Will I know the cost before starting care?
Absolutely. During your complimentary consultation, a doctor will review your situation, answer your questions, and explain expected costs before any care begins.
